For Waiver Recipients & Families
Are you a waiver recipient or family member ready to begin services with VC Integrated Services? To get started, please fill out our online intake form below. We provide services through the Family Supports Waiver and the Community Integration Habilitation Waiver. Once your intake form is received, we will coordinate directly with your case manager to begin the onboarding process.
